Heights Unlimited Vault Club also requires certain paperwork to be completed before each member’s first practice, including a signed parent/guardian consent form for each member less than 21 years of age. Please print out the following forms and bring them to your first practice of the season:

The Heights Unlimited Vault Club is also providing a manual to each member of the Club as a reference guide to the Club’s overall policies and procedures for the current indoor and outdoor seasons. The Club’s management feels that a clear and concise policy regarding the objectives of the Club and its membership’s responsibilities should be clearly defined. Therefore, in pursuit of this clarification policy the membership will hereby acknowledge these rules and policies by signing the consent statement agreeing to abide by the rules and regulations set forth by the Club. Consent forms should be brought to your first regular practice session.

The club allows its members to rent pole(s) to take to meets during the year. This allows the vaulter to always have the right pole available for competition. Members agree to the financial responsibility for the equipment borrowed; if a pole is returned broken or damaged beyond use then the borrower is responsible for its replacement.
Limit two (2) poles per session. All poles must be returned by the next practice session.
